1.  Description of proposed collaborative work by Choreographer and Composer/Musician/Sound Designer (C/M/SD)
2.  VHS tape of recent choreographic work
3.  Audio Tape of recent compositions by C/M/SDer’s
4.  Bios and/or Resumes of Choreographer and C/M/SD
5.  List of all performances two months before and two months after 21st Century performance
6.  Complete Contact information - name, address, phone, fax, email and web address for both Choreographer and C/M/SD
7.  SASE for return of materials (optional)

CRITERIA:
*  Choreograph a dance in conjunction with C/M/SD to be performed 3 evenings
*  Piece to be 6-9 minutes MAXIMUM in length
*  Group dances encouraged
*  Honorariums will be paid to Choreographer and C/M/SDer’s in accordance with ticket sales participation
*  Choreographers are available for 1 MANDATORY pre-production PHONE MEETING the week of June 24th to 30th AND dance is viewable by producers and technical staff LIVE on Aug. 18th or 19th in Marin
